To protect you and our staff, if you have traveled outside of the state in the last 14 days please contact the office to reschedule your appointment. We value your health and safety.

To login, all patients, please select Patient Information on the right side toolbar.

Patient Account Access window will pop up. 

Registration for your Secure Login is necessary to access your patient portal.

Register Now and click Sign Up if you have not created an online account. 

This will prompt you to create a user name and password.

• Password 8 characters.
• Only include numbers and letters. 
• No special characters such as #@*$&!

Register 3 days prior to your dental appointment.  This allows for the system to verify account access. You will receive an email when your registration is complete, allowing you to fill out the COVID-19 questionnaire.
